Permission Statement

I give permission for my child(ren) listed on this registration form to participate in the activities of Vacation Bible School at Bible Believers Baptist Church (BBBC), August 10–14, 2026.

I understand that reasonable precautions will be taken for the safety of all participants. In the event of illness or injury, I authorize church staff and volunteers to obtain emergency medical treatment for my child if I cannot be reached.

I release BBBC, its staff, and volunteers from liability except in cases of gross negligence.

I also grant permission for photographs or video taken during VBS activities to be used for church publications, presentations, website content, and social media unless I indicate otherwise below.

LIABILITY RELEASE: In consideration of Bible Believers Baptist Church, allowing the above child(ren) to participate in Vacation Bible School activities. I, the undersigned, do hereby release, forever discharge, and agree to hold harmless Bible Believers Baptist Church, its directors, employees, volunteers, and agents (collectively herein the “Church”) from any liability, claims or demands for accidental personal injury, sickness or death, as well as property damage and expenses, of any nature whatsoever which may be incurred by the undersigned and the above child(ren) while involved in Vacation Bible School. Furthermore, on behalf of my minor child(ren), I hereby assume all risk of accidental personal injury, sickness, death, damage, and expense as a result of participation in activities involved therein.

PHOTO/VIDEO PERMISSION: I give my consent to Bible Believers Baptist Church to use photo or video images taken of my child(ren) in church brochures, advertisements for the church, on the website, in social media, and in other church publications as they see fit. If I do not want the child(ren) photographed, I will indicate it in the form below.

I agree to hold harmless Bible Believers Baptist Church from any liability which may result from the use of said picture(s). This form will apply throughout my child(ren)s tenure at Bible Believers Baptist Church Vacation Bible School. **None of the photos will be for personal use. **
